The Birthday Card Program is so that every child in care on or before their Birthday receives a card by mail from someone. We ask that you give at least two weeks lead time. Please fill out the form below and be sure to include the following information in the “Message” box.
- First Name Only
- Gender
- Birth Month and Day Only
- Age Group: 1-4, 5-8, 9-11, 12 & Up
- Address Without Last Name
- Favorite Thing if Possible to Personalize the Card
- For Example: Star Wars or Dora
